12-04 MEETING NOTICE AND AGENDA-
Monday, December 5, 2011

1. Call to Order by Chairman
2. Accept and sign minutes of October 5, 2011and sign approved minutes of the May 10, 2011 Meeting by Nancy Miller. Note: Meeting scheduled for November 7, 2011 was cancelled by Mark Gifford.
3. Cemetery Business-Deeds
Sign a deed for Lot 245 grave 3 Centre Annex, Single grave (Phyllis Russell). This deed was not signed at the October 5, 2011 meeting.
4. Other Cemetery Business
* Discuss request by Donald F. McDonald to have the deed for lot 148 graves 1 thru 8 in the Agawam Annex split between himself and his sister Sarah DeForge allowing each to have 4 graves each. Ms DeForge presented herself previously as the executrix for this lot, when it fact, Mr. MacDonald is the executor.
* Discuss and vote on a request to be have Lot 658 graves 1 thru 9 in the Centre Cemetery transferred from his deceased grandfather, Delbert C. Keyes to his grandson, Dana C. Keyes and having his daughter, Holley Gardiner as lot representative.
No response has been received from Willis E. Maxim Sr. pertaining to two previous transfer requests listed below. (Letter to Mr. Maxim is included in meeting packet)
a. To transfer ownership of Lot 22 in Agawam Cemetery to Willie E. Maxim, Jr.
b. To transfer ownership of Lot 21 in Agawam Cemetery to Mark B. Maxim.
Review request for a headstone to be placed on Lot 249 grave #6 in the Centre Cemetery for Kirah L. DiRienzo. There is already a stone there for Christina DiRienzo and her stone would need to be moved and adjusted to allow a stone for Kirah DiRienzo.
Review and discuss rules and regulations regarding gravestone cleaning.
Discuss Centre Cemetery wall and it’s condition after its repair. This wall is incomplete and the vendor is not longer approved. The headstones were repaired wrong and are now inappropriate and unacceptable.
Discuss Agawam Cemetery’s stone wall, its condition and methods to repair.
JC Engineering, the firm hired to begin surveying the Agawam Cemetery and the expansion should not be conducting business with the Town. There are client issues brought to the Town by JC Engineering.
