Street Census: Forms were sent out to all households in January. All full time residents must fill out and sign this form. All members of your home should be included.
The annual street census is required by the State of Massachusetts General Laws and failure to sign and return this form whether or not you have any corrections, additions or deletions, will result in removal from the active voters list.
