Richard Stevens called the meeting to order at 9:05 a.m.

Request for “informal” Design/Plan Review to discuss
Proposed two-story addition to existing facility______
Karen Gardner, Rudy Hunter, Katy O’Connell and Mike McGrath appeared. Karen Gardner provided a brief description saying the site at 107 Commercial Street is just 10,000 square feet. Currently the Health Care Center is extremely successful with 12,000 patients. She said they continue to register between 180-200 patients each month; they expect growth from 12,000 to 18,000 patients and that would be maxed out for that site. Karen Gardner said primary care services will be expanded…12 exam rooms will be increased by another 18; 3 dental labs will be increased to 10. They will add some additional support services: a small in-house pharmacy for patients only to offer discount pharmacy to patients without insurance; expanded radiology would be added to this building and they would like to plan for
mammography but not until several years down the road but would do X-Ray services. Karen did a quick review of the Plan:
- Add to current building towards Route 28.
- Have 2 connections—one for employees and one for patient traffic.
- 2 entrances—Patients can flow through existing entrance or also enter through ground level. The thinking is that through time the lower entrance would be primary location.
- Adding 9 additional handicap spaces in front of the current entrance.
- Building would be over 22,000 square feet so they’ve engaged an attorney and traffic engineer and they will provide engineer’s report to the Town.
- They elected not to add a curb cut. In lieu of a curb cut they considered a meandering sidewalk that runs from Route 28. Preliminary discussions with engineer indicate emergency exit off 28 not necessary now.
- In total about 164 parking spaces. This is okay since parking they added last year does the trick. Tom O’Hara questioned parking and Karen Gardner said currently they have roughly 25 open spaces now and additional 63 new spaces at the front of the building should be sufficient.
- Karen Gardner said they’ll be talking to Cape Cod Commission about some of the open space issues.

- Town had been discussing fire truck and construction vehicles going down to lower lot…Karen said their engineers say the retaining wall is sufficient.
- She said the Fire Department has a template to overlay on their schematic and they did and it looks fine. She will send e-mail to Fire Department. Chief Baker noted that he still needs the 1:20 scale of the site plan; Karen will request that.
The building schematic was reviewed:
- Front center is glass area and the stairwell is there. They are trying to achieve silver LEED rating so they are looking for any opportunity to get natural light in the space.
- Elevator is in center of building right at the connection of the 2 buildings.
- More of a square box than present building; but still nice looking building.
- First floor is 19 feet high; second floor is 10 feet—29 foot building height.
- Tom O’Hara suggested multiple types of roof lines be considered.
- Building chemical free—all digital.
- Steel Studs. Sheathing not decided. Chief Baker suggested the hardy board used on Fire Station 2. It is extremely low maintenance. Catherine Laurent added that it is made from recycled material and may be added to their LEED list.
- Pharmacy drugs will be scheduled drugs. Chief Baker pointed out security requirements differ so it has to be looked into to know what is required.
- Route 28…Chief Baker suggested if curb cut or roadway done they could design it to look like a sidewalk but it could be support grass that would withstand fire truck access. Catherine Laurent pointed out that curb cut is under Mass Highway. Tom Fudala stated that Commercial Street was created to make sure no curb cuts on Route 28. It was suggested by applicant that if they make pathway there then plantable blocking could be put in for fire truck access. Catherine said with the issues we’re having on Industrial Drive it would be good to have some additional access. Tom O’Hara commented that Industrial Drive should be cleaned up. Rich Stevens said he spoke with one of the owners and they are inclined to do something. Mike McGrath said this is public safety and he would rather put plastic inserts in rather than
grass. He said he would keep the same curb cuts but make it so that you could easily drive up.
- Chief Baker stated that with the new building access improves.
- Rich Stevens asked whether there will be another retaining wall. Karen Gardner said not designed yet.
- Bob Nelson commented that they were told one road would disappear but with additional parking that didn’t happen.
- New building will have fire doors and sprinklers. Chief Baker said because of size of the building and metal studs, Applicant may need to consider bi-directional amplifier for fire department radio system. Chief Baker suggested they put this on their electrical engineer’s list.
- They are going to Planning Board after Cape Cod Commission review.
- Tom Fudala asked about wastewater treatment. Mike McGrath explained system. Mike McGrath said they have to have total loading below “5”. He said system is mechanical in that waste water is doused with pump. Tom Fudala asked if it’s a variation of the rock system. Mike McGrath said all systems look the same now.

CONCLUSIONS:
- Scaled architectural Plans to be submitted.
- Show second means of egress from second floor.
- Show all different floor layouts.
- Basement may need second egress from that floor, depending on layout.
- Building Code requires narrative so document should be supplied with narrative.
- Provide Certification from engineer that the retaining wall that was designed and approved was built and then Town engineer will review.
- Catherine Laurent interested in seeing applicant’s proposal for recycling. Catherine said that could be addressed in the narrative or on the site plan.
- RTA looking at putting bus shelter in. Catherine Laurent asked if they were planning on installing a sidewalk from their facility to South Cape. Response: “no”, but they will check into this.
