M. Johnson called the Board of Health Meeting to order at 5:40 PM
SEPTIC PLAN REVIEW: 564 Quinapoxet Street, Septic System Upgrade for Gail and Bob Claflin. This septic system was reviewed and approved September 1998.
The septic system needs a current Board of Health approval for the local upgrade approval for the offset to groundwater from 4 feet to 3 feet. A new Local Upgrade Approval form and letter requesting the variance to groundwater offset needs to be submitted to the Health Agent for approval. The Board also wanted verification of the designers date on the signature stamp.
M. Johnson voted to approve the septic system upgrade on the condition that a new LUA form and variance letter are submitted. B.Pennino seconded. All in favor. Approved.
Documents presented during review: Septic Plans, 564 Quinapoxet Street, Lot 19, Map 89 / Parcel 19, for Gail Claflin, dated July 29, 1998.
