The Board of Health Meeting was called to order at 5:05 PM
SEPTIC PLAN REVIEWS:
Lot 4 Fisher Road: Lew Reed, Wachusett Environmental for Thahn Nguyen. Review of plans previously approved on January 9, 2006 for Board comments. The applicant has relocated the house as well as substantially increased the size. The Board recommended adding pump chamber calculations to the plans. No Action.
Lot 2 Mixter Road: Andrysick Land Surveyors for John & Wendy McManus. The applicants submitted plans for a new system.
Conditions:
Two (2) additional deep holes, and observation, are required per 4/21/06 revised Title 5
regulations.
Must comply with new Title 5 regulations including but not limited to:
magnetic tape needed on all system components
inspection ports are required
risers over d-box and septic tank if finished grade is greater than 9”
The SAS must be properly staked by the surveyor/engineer.
The SAS must be installed exactly to plan.
The well must be staked by the surveyor/engineer.
M. Waite motioned to approve with conditions 1 – 5. M. Ricker seconded. Vote 2-0, approved.
OTHER BUSINESS:
Outdoor Wood Fired Boilers: Bruce McKay from the Fire Dept. has requested to be put on the agenda at a future meeting to discuss.
FAILED SEPTIC SYSTEMS:
450 Sterling Road: Denise & Darci Silva. The system was approved September 13, 2004, and installed October 2004 by Holden Sanitation. This was for a repair/upgrade by Holden Sanitation.
Darci Silva, owner, addressed the Board, explaining the problems they’re having with the system. Has tried to contact Jim Morin, Holden Sanitation, numerous times with no response. The system was pumped in January 2007, and twice in July 2006 by Holden Sanitation.
Jim Morin, Holden Sanitation, addressed the Board and explained the system which uses Cultec Chambers. This is called ‘premature failure’. J. Morin stated that 4 soil samples were taken and water samples were taken as well. He found that one section of chambers was not accepting effluent, because of a collapsed area. It can be fixed by exposing the area. J. Crouse stated that the samples were not witnessed by anyone from the Board of Health. J. Morin said that he will put together a package of all his testing/material for the Boards review. R. Mizereck pointed out that the owners are under orders to correct the system. The original order was dated February 9, 2007. The Board agreed to extend the order 30 days from March 12, 2007. J. Crouse, or an
agent, will witness and obtain the soils again at the same time that the site inspection is conducted. Cultec representative will be present also.. J. Morin agreed to be at the site inspection on Wednesday March 14, 2007 at 9 A.M. with R. Mizereck as witness.
J. Morin stated that a system needs to be pumped anytime liquid is coming out of the ground. He agreed to pump this system March 13, 2007 free of charge. And if the first row of chambers has failed, it can be fixed in 3-4 days. He is to notify the Board of Health before doing any work.
786 Princeton Street: Richard & Blanche Farrell. The plans were approved February 23, 2004, and the Certificate of Compliance issued February 17, 2005. This was for a repair/upgrade by Holden Sanitation.
Blanche Farrell, owner, addressed the Board regarding issues with their system, along with a tank cover that was broken when Holden Sanitation pumped the tank. There is a small child living at that address. B. Farrell tried many times to contact Holden Sanitation with no response. The system has not been recently pumped.
Jim Morin, Holden Sanitation, addressed the Board stating that he used the same as a failed system, and no collapsing was found. He said the system was starving for oxygen and went anaerobic. There are no vents, except the observation vent. The Mr.Alan Barbaro of Infiltrator Systems Inc. discussed the system and the interface failure. Michael Cove, with the Infiltrator Systems, would like the Board of Health to witness the sand samples. Everything should be done and witnessed by a BOH rep.
Randy Mizereck informed the Board that this was an anarobic (without oxygen), and it can be changed to an arobic sytem, and recommended air flow vents be added to the system (vent the SAS).
The Board wants a BOH agent to witness the soil samples, with a full evaluation of the system. An unbiased, second opinion by an engineer was recommended by the Board. A BOH rep will meet on site Friday, March 9, 2007, with a rep from Infiltrater System.
The Board agreed to modify the original order 30 days from March 12, 2007. J. Morin was told to replace the
septic cover right away.
J. Crouse will be present for the site inspection and witness and obtain the soil samples again.
J. Morin stated that he would assist both system owners in making assessments of why the systems went into premature failure and follow up with the subsequent repairs. He would provide his equipment and operators in making those assessments at no cost to the system owners. The Board instructed him to maintain both residences on a pumping schedule to prevent the systems from backing up into the homes. Mr. Morin agreed to this and agreed that the pumping would occur after the site inspections. J. Morin has also agreed to get emergency dig safe approval.
