Kenney, 45 Beach Road (2-3) Reid Silva presented a septic system tie-in for a guest house that is being relocated.  The Board approved the application, contingent on receipt of a septic plan showing check valves in the system.

Flanders, Flanders Lane (26-31.2) The Board approved the well construction permit application.

Beach Plum Inn: Post-Season Review:  The Board reviewed and signed a letter to Beach Plum Inn owners Robert and Sarah Nixon and chef Chris Fischer, outlining Board questions and concerns.  The Board directed Marina Lent to work with Chris Fischer to thoroughly address these concerns prior to the opening of the 2015 season.

LoRusso, Chilmark Tavern , 9 State Road (30-85)  Kent Healy and John Clarke presented an application for replacement of the PWS #4062002 drinking water well serving the Chilmark Tavern.  A permit application to drill an inactive well to hold in reserve for the Chilmark Store on the abutting property was withdrawn.  John Clarke said he would attempt to repair the existing well for the Tavern, but had submitted a well replacement application in case it was not possible to repair the existing well.  The Board noted that approval from the Department of Environmental Protection and the Chilmark Conservation Commission will be required for this project to proceed, and approved the application, contingent on informing the Board of the identity of the Public System Operator responsible for this system.

Sbriglio, 51 Putnam Road (8-7) The Board reviewed applications for a new well and septic to serve a proposed five-bedroom dwelling.   The Board approved the applications, with the requirement that system designer Chris Alley confirm soils prior to placement of fill.

Muglia, 3 Blue Barque Road (17-38.1) Chris Alley presented a proposal to expand a septic system to accommodate renovations to the dwelling which will add a bedroom.  The proposal includes a new leaching system and 2,000 gallon tank.  The bottom of the proposed tank and pump chamber will be close to, if not in, high ground water, and excavation and fill will be required for the leaching system.  The Board noted that construction will have to be extremely accurate to the plan, and required that the as-built plan for this system should certify elevations of the system.

Walker, 17 South Ridge Road (18-36) Chris Alley presented septic construction permit application to expand the system to accommodate an addition.  The proposed plan will replace the 1,000 gallon tank with a 1,500 gallon tank and build a new leaching system in the location of the existing system, as soils are challenging.  The Board approved the permit.

Leopold, 11 South Ridge Road (18-37) The Board reviewed a septic system permit application submitted by George Sourati to construct a new, 6-bedroom system to accommodate a renovation of the existing four-bedroom house that will add two bedrooms. The Board approved the proposed plan with the requirement that the engineer will inspect the bottom of the excavation in the leaching field prior to placement of any fill.  

Wallach, 5 Red Wing Road (25-110) The Board reviewed renovation plans presented by Jamie Weisman to confirm that the proposal is within the capacity of the existing septic system, and to confirm that the existing house currently has three bedrooms, although the bedroom count will be temporarily reduced to two in the course of the renovation.  In light of the fact that the system is old, and that there are no records on file of the original construction plan for the system, the Board suggested that a new, three-bedroom Title-5 compliant septic plan be produced for the property which can be held in reserve until needed.

Shae, North Road (8-45) The Board approved  a request for determination of un-buildability on the basis of the extensive wetlands throughout the property.

BOH Budget Process FY’16  Marina Lent reminded the Board that we will prepare the FY’16 draft budget to convey to the Finance Committee no later than January 1, 2015.

Treitman, 15 Periwinkle Lane, (32-63.3) Title 5 Official Inspection Report – passes: the inspection report notes that the leaching pit is located 50’ from an eroding coastal bank.

Food Establishment Permit: The Board noted that a Food Establishment permit for the winter season has been issued to The Food Truck, owned and operated by Josh Aronie.  The Board requires that greywater from the handwash sink on board the Food Truck must be trucked down to the Wastewater Treatment Plant in Edgartown and cannot be disposed of in the Homeport septic system.

Kelly, 28 Black Point Road (17-27)  The Board noted receipt of an as-built letter for septic tank installation.

Wilmont, Brickyard Road (9-2.5) The Board noted receipt of a well construction report.  Drinking water from the well passes potability tests.

Sunrise Partnership, Ltd 26 Greenhouse Lane (33-37) The Board noted receipt of a well construction report for a replacement well.  Drinking water from the well passes potability tests.

Temporary Food Permits have been issued to: Chilmark School 10/24/2014 and 11/10; Chilmark Community Center, Community School Lunch, Robert Lionette (chef);  Farm, Field, Sea, 10/25, 26 Nevette Previd;

Yaffee, 41 Tabor House Road (13-27.1) A revised septic plan showing Presby certification required for installation was received at the Board of Health office.

Silver, 33 Beetlebung Road (20-34)  The Board of Health office has received a Title 5 Official Inspection Report – the inspection passes.
