The minutes of November 15, 2006 were approved.
Perc Season will open December 7, 2006.
All Phaze Excavation B The Board approved this disposal works installers permit.
All Island Flu Clinic -- Sylvia reported that the Vineyard Nursing Services purchased $10,000 worth of private vaccine for the flu clinic. The state provided only 20% of the flu vaccine. Roster billing can only be done on private vaccine for Medicare patients only. The agency does not bill for state provided vaccine. The vaccine and administrative costs are approximately $35 per shot. The nurses were paid $25 per hour. The total expense for nursing services was $1,150.
Martha Flanders (30-79.3) B The continuation of this disposal works hearing was opened at 5:30 PM. The engineer submitted USPS return receipt cards. The calculations for this septic design are for two bedrooms, but this upgrade is only a one bedroom residence. A deed restriction will be imposed to limit this dwelling to one (1) bedroom. This design requires the following variances; Title 5 - 1) A reduction in separation between leaching area and high ground water from four (4) feet to three (3) feet. Chilmark variances B 1) A reduction in separation between leaching area and property line from 30 feet to 10 feet (A.P. 30-81). 2) A reduction in separation between leaching area and wetlands from 150 feet to 50 feet. 3) A reduction in separation between leaching area and high ground water from five (5) feet to three (3) feet. 4) A reduction in separation between property line and leaching from 30 feet to 10 feet. The Board wants the skirt of the breakout design to be
heavier than 6mm poly. Sylvia researched Chilmark Chocolates and confirmed that they are not a public water system. The Board requested a new plan with the separation variances listed on the plan. The Board approved this disposal works system with the following condition, it must be installed within 90 days (March 6, 2007). The Board also approved a well permit application for this parcel. The existing well must be abandoned.
Roseline Glazer (21-78) B The Board approved the abutter letter and reviewed a new plan to be presented at the hearing on December 20, 2006.
Michael Zane (8-31) B The Board approved this disposal works permit which had expired. It was originally approved on April 19, 2000. There are no changes in the design.
Rosalie Hornblower (35-1.6) B The Board approved this disposal works application with the following conditions; 1) The kitchen in the studio must be removed when the certificate of compliance is issued on the new house. 2) The well must be tested for potability and yield. The septic construction permit will be held until we receive the well report.
Jaqueline Schuman (32-58) B This is a septic repair permit application. The work is to be done by Mark Peters, working for Kevin Peters. The Board has requested the following, 1) schedule 80 pipe must be used, 2) The Board wants to be on site during the repair, 3) They want to see the old pipe, 4) and they would like a more detailed drawing showing the site and location of components. The Board will hold off on approval until they see a new drawing.
Michael Cooper (11-16.1) B The Board did not approve this well permit application at this time. They want to know if the well is sited 30 feet from the property line, and why the well is being replaced.
Dennis Jason (20-98.1) -- The Board did not accept this Aas-built@ letter and drawing because the septic system installed is not in compliance with the septic plan #04-38B, submitted to the Board of Health and approved on January 19, 2005. The Board instructed Sylvia to send a letter to
Kent Healy, requesting a new revised site plan and his attendance at the next meeting.
The Board instructed Sylvia to ask Tim Carroll what the status is on the phone line at the landfill.
The Board reviewed a letter from Leslie Weiss regarding the problem of dogs defecating on the public beaches. The Board has instructed Sylvia to write a letter to Mrs. Weiss informing her that the problem is a Park and Recreation Department issue.
Sylvia reported information obtained regarding the seat capacity at the Home Port Restaurant. The Board asked Sylvia to contact Brian Dudley at the regional DEP office and ask him where to find this information in Title 5. If that is not available, they would like Mr. Dudley to put this information in writing on DEP letterhead.
