Chilmark Board of Health Minutes

Attendees: Mike Renahan, Katie Carroll, Jan Buhrman, Chris Alley, Peter Wells
The minutes of October 18, 2006 were approved.
Schuelein Nominee Trust (29-23) B Mike approved a well installation for this parcel. Jeff Kurth is the well installer. This Permit application is being held because the well is not sited on the plan.
Howard Feinstein (19-69.1) B This well permit application was approved.
Girl Scout Council of S.E. Massachusetts (18-20) B The Board approved a well permit application for this parcel. They are increasing the separation from the abutter=s well. The septic plan on file is to be revised, but they are still waiting for ZBA approval. The
proposed well location was staked by Chris Alley. The existing well will be properly abandoned and the well completion report mustl be received prior to approval of a new septic location.
Heidi Riggs (25-7.4) B The Board approved the relocation of this well for the purpose of opening more septic location possibilities. The well was staked by Peter Wells.
Martin Hale (35-44 & 45) B The current septic design is for six (6) bedrooms. The owner currently has three (3) bedrooms and a loft in the house, and has proposed a detached bedroom over the garage. This disposal works application approval will be held for a site visit for a bedroom count prior to approval. The schedule 40 sleeve must be upgraded to a schedule 80.
Kevin Peters B The Board approved a Disposal Works Installers Permit.
The Board noted the following septic inspection reports:
Seth Mosler (12-69) passes Adell/Terney (24-204) passes
The following invoices were submitted and approved:
Cingular phone at landfill $ 33.70
Health Inspector travel expense 26.35
Visiting Nurse Service (adjusted invoice for September 2006) 573.75
