Chilmark Board of Health Minutes

Attendees: Mike Renahan, Jan Buhrman, Sylvia Yeomans
Meadow Wood Farms, LLC (19-56.2) and (19-56.1) B The Board does not want two wells on each property. The well driller will speak to the owner prior to any further process of these applications.
Forbes Smiley (20-41) B The certified mail that was sent to Mr. Smiley was returned by the post office. The Board signed an order letter to Mr. Smiley giving him 60 days from receipt of the letter to make the necessary repairs, as described relative to the completion of installation of the septic system. This letter will be delivered by hand by the Constable pursuant to 310 CMR 15.026 (2) (a) and (b).
Chilmark Road Race B The Board reviewed the Sanitation and Trash Disposal Plan submitted by Hugh Weisman for this event. There will be six porta-potties at the start, two at the finish, and two at the Community Center. He enclosed a map showing the locations of the porta-potties, refreshments, water table, trash receptacles and the ambulance.
Krista Martin B The Board approved a license for Ms. Martin to conduct Massage in the Town of Chilmark.
The following Aas-built@ letters were received:
David Damroth (35-6) B John Kenne Excavation, Inc. installed this upgrade to an existing failed cesspool (to be abandoned) serving a four bedroom cottage located on Blacksmith Valley Rd.
Bruce Bartels (8-68) B DECA installed this upgrade to an existing failed system serving a three bedroom house. The existing 1,000 gallon septic tank was replaced with a 1,500 gallon tank as part of the upgrade.
Michael Urban (30-36) B John Keene Excavation, Inc. installed this upgrade of a cesspool serving a three bedroom house. The cesspool will be abandoned and the new septic system includes a 1,000 gallon pump chamber.
