Jay Grossman (8-40) B The Board approved this disposal works application.
Walter Meinelt (17-8.6) B The Board agreed 2 to 1, to allow an out-of -season soil evaluation for this parcel. Mike Renahan voted against.
Meinelt (Sue Whiting 23-1) -- Mr. Healy requested an out-of-season soil evaluation at this parcel, the Board voted not to allow this soil evaluation at this time.
David Meen (21-21.1 & 22) B This disposal works application is a revision to permit #01-05 which was extended to March 7, 2005. The engineer requested approval of this revision without a new hearing. The Board denied his request and has scheduled a hearing for July 6, 2005 at 6:30 PM. Rusty Walton has the following comments regarding this application. He believes there may be some discrepancies between the septic plan and the landscape plan that was presented. Mr. Walton listed the following; 1) The two plans do not
match. Part of what appears to be a connection between the garage/guest quarters is labeled as Aplanter 66.0". 2) The calculations are for a three bedroom house, but the plan the Conservation Committee has, show only (ground floor) two bedrooms. Is there another bedroom over the guest room/garage? Is there more living space either over or under the master bedroom? 3) There appear to be three connectors to the septic system, one from the kitchen end of the Agreat room@, one from the master bath, and the third from the garage/guest quarters. As there is no bathroom shown for the guest room, it is assumed that there is a bathroom on the second level. There appears to be a door between the garage space and the guest room, and there also appears to be a fireplace against the garage.
Leo Buzzuto (11-28.15) B The Board approved this disposal works application which expands the leaching area to increase the bedroom count from four bedrooms to five bedrooms.
Trina Kingsbury (3-1) B This disposal works hearing was opened at 6:50 PM. The following variances are being requested; 1) A reduction in separation between leaching area and property line from 30 ft. to 10 ft.,
2) A reduction in separation between leaching area and ground water from five (5) ft to four (4) ft.,
3) A reduction in separation between leaching area and wetland from 150 ft. to 70 ft. This septic design is an upgrade from a cesspool. The Board is requesting the following; 1) The septic design must include an alarm for the pump chamber. 2) Water tight tanks must be used, and the tank capacity must be specified on the site drawing. 3) Pump specifications must be included on the site drawing. 4) The drawing must include a note indicating that the old cesspool is to be abandoned. 5) All variances must be listed on the site drawing. 6) The site drawing must include installation instructions for the installer.
The engineer will get a meeting date from the Conservation Committee, if the date is prior to the next Board of Health (BoH) meeting, the BoH will send a letter to the Conservation Committee explaining the BoH is approving this design, but waiting for more information. This disposal works hearing is continued to
June 15, 2005 at 6:45 PM.

Lorriane Holdsworth (33-87) B The Board approved this guest house disposal works application with the following condition; any waste water coming out of the building must be attached to a septic system.
Maxwell McCreery (24-206) B The Board approved this disposal works application.
Thomas Porter (26-34) B This disposal works hearing was opened at 7:35 PM. This septic design requires the following variances; 1) A reduction in separation between well and leaching facility from 150 feet to 105 feet., 2) A reduction in separation between leaching facility and property line from 30 feet to 15 feet.,
3) A reduction in separation between leaching facility and wetland from 150 ft. to 72 ft. 4) A reduction in separation between ground water and leaching area from five (5)ft. to four (4) ft.
The Board asked for a new septic design with the following; 1) Use of an impervious barrier, 2) Locate the leaching area next to the house, 3) This will require moving the well, and a reduction in separation of leaching to wetland. This hearing will be continued on June 15, 2005 at 6:15 PM.
Sheriff=s Meadow Foundation (22-5.3) B The Board approved this disposal works application.
Reid Badger (30-47) B Reid Silva reported that this septic system is deemed a fail because the well is too close to the abutter=s leaching area. He informed the Board of Health that a septic inspection report is forthcoming. The engineer is requesting an out-of-season perc test due
to the failed system. The Board approved this out-of-season perc test because it is a public health hazard.
Allan J. Arffa (26-28) B The Board approved a one-year extension to disposal works permit #02-24. This permit is valid until May 15, 2006.
Frank LoRusso B It came to the attention of the Board , that Mr. LoRusso had a pump-out done at the Cornerway building without a permit. Mike signed a letter to Mr. LoRusso explaining that a pump-out permit is required prior to performing any pump-outs.
Pepperbush Farm (13-6.1) - The Board signed a letter regarding the Form C Application. The Board disapproves the eastern most building envelope site, but approves the rectangular building envelope located at the westerly portion of the property because the Board believes this is the only part of the property that will allow for safe conveyance of septage to the leaching facility.
Landfill Closure B Mike reported that the landfill closure was approved by DEP but he cannot locate the permit to dispose oil. We will try to open on June 13, 2005, however, we still do not have the proper dumpsters, the Refuse District is discussing where to get them. We also need signage but have not determined what it will be. Mike sent a punch list to Nate Weeks and Lawrence Lynch.
The Bite B Food handling procedures were discussed and it was determined that Sylvia will review with the owner, proper food handling and supervision of the staff to insure no cross contamination.
Robert Sequeira B The Board denied this application for a disposal works installer=s permit. The application was denied at this time because the Board does not feel he has the equipment necessary to do the job.

Norman Willard (30-66) B The Board did not approve this well permit application at this time. The Board would like to know why the existing well can not be used for this parcel.
