BOARD OF HEALTH MINUTES

Meeting held at Town Hall on June 9, 2009 6:30 pm
Members present: Chris Rogers, Christopher Slade, Lisa O’Leary

265 SAWYER RD. LOT 3 – PAUL D’ANNELLO
The Board’s letter dated 12/29/08 stated that the Board required the placement of two boulders or another appropriate physical barrier be placed between the well and the driveway and a revised as built that shows the driveway and the barrier location no later than 5/30/09.  – Bill Brookings and Chris Rogers inspected the property and found that there is only one boulder instead of two the Board requested.  The Board agreed to send a letter requesting the additional boulder and the revised as built showing the driveway and the barrier location.

LOT 2 SAWYER RD. – JOHN FINN SEPTIC PERMIT TRANSFER
Information regarding ownership was submitted by Andrea W. McCarthy, Esquire and the Town of Bolton Assessors.  The Board wants more information they would like to get some information from Larkin Lumber prior to approving the septic permit transfer.

233 WATTAQUADOCK HILL RD.
Information submitted by Casaceli Trucking for discrepancies between an inspection report dated 6/24/99 and his report dated 4/9/09.  The Board reviewed the information submitted and considered the report complete.  Bill Brookings explained that the testing did not support the White Knight rehab system, so a new system will be going in.

NORTH CENTRAL-FRANKLIN COUNTY TOBACCO CONTROL ALLIANCE
Retailer Compliance Check – Mobil Station and Twin Springs Golf Course were missing the Cigar Warning Sign.  Signs were given at inspection.  They have been sent warning letters and if the signage is missing age they will face a $100.00 fine.

COMMUNICATION REGARDING FLU AND THE SCHOOL DISTRICT
Chris Rogers discussed the recent directive regarding sending a child home for 7 days if they have 100+ temp and one other flu like system.  He was not aware of this directive and was getting questions from town residents regarding it.  The Board discussed that this is the type of information the School District needs to communicate with the local board.  The Board agreed that this will be discussed at a future meeting with the School District.

DOCUMENTS REVIEWED:
Bolton Orchards septic filters changed 6/2/09 and water meter reading 159282
21 Ledgewood Cir. Lot 23 - Pump specs submitted for the finished basement by Michael Fox/Kendall Homes.  The Board wants Bill Brookings to check out the pump specs.

INSPECTION REPORTS REVIEWED:
32 Frye Rd. - last occupancy 12/08 – The Board requires re-inspection 6 months after occupancy.
144 Fox Run Rd. - last occupancy 11/08 – The Board requires re-inspection 6 months after occupancy.

SEPTIC PERMITS:
336 Sugar Rd. – Ed Laquidara
Board signed the permit and requests a revised plan for variance free system.

BUILDING PERMITS APPROVED:
49 Berlin Rd. - Rob MacGregor and Ed Plante Jr. were present.
The Board agrees that total of 7 rooms 3 bedrooms – The existing house will be renovated and the existing addition on the house will be removed and rebuilt.  There is a new wall that will replace an existing wall that is less than the local 20 ft offset.  The Board discussed the fact that the well is 6 feet from the existing foundation.  As it is an active well for~the main house, the Boards is ok with is as long as they can guarantee its safety during construction.  The Board requires the well to be brought to grade as part of the construction.  The Board recommended that they excavate the old foundation, form, pour and strip the new foundation, backfill, then bring the existing well casing to 18" above finish grade.

1108 Main St. – install a sign.  Steve Lamontagne was present.  The Board reviewed the sketch of the location of the sign and approved the building permit.
116 Sawyer Rd. – Ken Nicewicz construct a barn for storage & small retail sales.
