BOARD OF HEALTH MINUTES

Meeting held at Town Hall on December 15, 2009 6:30 pm
Present: Chris Rogers, Laura Andrews, Christopher Slade, Bill Brookings, Lisa O’Leary

83 QUAKER LANE D BOX
Larry Gogolin and David Poor were present.
Larry Gogolin explained that David Poor his son in law installed the D Box.  David Poor explained that he did replace the D Box after his installers license had expired and ask Don Morris if he could do the replacement under his company.  Don Morris agreed however, he did not renew his license before it expired either.  Bill Brookings explained that the problem is that the D Box was installed without a permit by an unlicensed installer.  The Title 5 inspection report received by the Board of Health indicated a new D Box, however, there was no record of a permit being pulled for the installation.

After a brief discussion, the Board agreed to allow the permit to be processed by a licensed installer.
David Poor said that DJ Morris is currently licensed and will continue the permit application process.  Bill Brooking explained that once the permit is issued he needs to call for an inspection.  

The Board requires the D Box to be permitted, inspected and to obtain a Certificate of Compliance.

188 STILL RIVER RD. GEOTHERMAL REQUEST
Bill Brookings said that he has not heard back from Rick Starodoj who made an initial request about a geothermal system.

CAMP VIRGINIA EMERGENCY RESPONSE PLAN
The Board agreed with the plan to shut down in the event of an emergency.

PUBLIC SAFETY BUILDING
The Board said they had reviewed information they received regarding the removal of the underground storage tank and the soil samples being tested from underneath it.  The Board agreed that they will keep abreast of the situation.

SUNSET RIDGE AS BUILT
The Board noted that this was only the presby as built plan not the whole system.  The engineers will still need to submit the DEP water supply and the full as built.

SEPTIC PERMITS APPROVED
440 Harvard Rd. permit extension requested, Bill Brookings explained the property is being sold but the permit is due to expire.  The Board granted the extension and the Certificate of Compliance must be obtained by 2/27/11.

BUILDING PERMITS APPROVED:
737-738 Main St. – addition approved by Bill Brookings

INSPECTION REPORTS REVIEWED:
626 Main St.  – The Board reviewed the reports for the 3 systems that serve the Salt Box, one system failed.  The Board will issue a failure letter and requires the tight tank must be abandoned in accordance with Title 5 15.354.
129 Oak Trail – The Board reviewed the inspection report and found no failure criteria

MINUTES APPROVED:
The Board reviewed final minutes for the Board of Health meeting on August 11, 2009.  Laura Andrews made a motion to accept the minutes seconded by Chris Rogers, all in favor (Laura Andrews yes, Chris Rogers yes, Chris Slade yes) to accept and sign the August 11, 2009 minutes.
