BOARD OF HEALTH MINUTES

Meeting held at Town Hall on December 9, 2008 6:30 pm
Members present:  Mark Sprague, Andrea Gilbert, Chris Rogers, Bill Brookings, Lisa O’Leary

265 Sawyer Rd.
Paul D’Anello was present.  The Board requested that Mr. D’Anello come to the board meeting to discuss why his existing well and driveway location are not in compliance with the approved septic system design plan number 3656D-SDS-3 dated 5/24/06 revised 7/24/06 by Ducharme & Dillis Inc.  Mr. D’Anello explained that the contractor that did the paving of his driveway did not pave directly on the gravel.  The width of the driveway is 15 ft and widens out to 17 ft.  He explained that he would rather not tear up the driveway and leave it as is.  The Board did note that they did already give him a variance but the driveway is still not to plan.  Bill Brookings explained that the 25 ft offset is basically for well head protection.  Mr. D’Anello said that winter is here and can he install a barrier in the spring.  The Board explained that they made a concession with the well originally and are only asking that a few boulders be placed to protect the well head.  Mr. D’Anello said that he would like to match a wall he did on the side of his property.  Mr. D’Anello said that he can definitely get this done by the end of May.  The Board agreed to allow Mr. D’Anello to install a physical barrier and submit an as built plan showing the barrier on or before May 30, 2009.

Larry Ducharme and Duane Henry were present.
Mark Sprague recused himself from the discussion as he is an abutter.  Larry explained there is an issue with the testing and thought about doing a sieve analysis.  Although there is testing to meet title 5 regulations but not to meet the Bolton regulations.  Larry said everything was a disaster he paced off the test holes and said something was not right.  He scaled the tree, the tree is in the right spot on the plan but everything else is off 30-35ft.  Larry explained that the plan was actually done by another company but the problem was he super imposed his test holes on their plan.  Larry said he must have used a different scale than what they used, so the test holes are not actually plotted right.  Larry said he made a mistake plotting but everything else is correct.  Larry will be going out and checking all the test holes.  The plan is that Bill and Chris will go on site with Larry on Friday morning at 9:30 am to confirm the test areas.

The next step will be an official submission of a plan, a presby system and variances are percolation rate over 30 and reserve area off the wetlands.

The Board approved the proposed plan with a long sewer line by homeowner restriction.

INSPECTION REPORTS:
571 Wattaquadock Hill Rd. the Board reviewed the report and noted last day of occupancy was two months prior to the date of inspection and page 1 of the report indicates that the distribution box needs to be replaced.  The Board requires inspection 6 months after occupancy and a permit to replace the distribution box.  In addition, Northboro Septic needs to resubmit page 5 of the report that indicates as built plans were not available but a previous inspection report dated 5/23/01 indicated that as built plans were available.
39 Drumlin Hill Rd. – The Board reviewed the report and requires Northboro Septic to resubmit a revised page 6 that correctly labels the design flow.
21 Harris Farm Rd. – The Board reviewed the report, no failure criteria was noted.
119 Sugar Rd. – The Board reviewed the report and requires inspection 6 months after occupancy.

ACTION/DISCUSSION:
Annual Report – The Annual Report is usually done by the Chairman of the Board and is due by January 20th.  The Board agreed that Mark Sprague will prepare the Annual Report.

DEP and Emerson Well
Mark Sprague had no update to report.

Review of Public Safety Building & Library Plans
The Board reviewed the plans no comments were noted at this time.

Adult Wellness Clinics & Flu Clinics
The Board voted to approve Wellness Clinics conducted by the Town Nurse be only one day at the senior center and one day at Town Hall.  The Board also voted to hold the Flu Clinic at the senior center for seniors only and at Town Hall for everyone else.

Title 5 re-inspection 6 months after occupancy is due – The Board voted to send a letter.

LICENSES:
The Board reviewed the Tobacco License letters and the new registration form.  The Board authorized Lisa O’Leary to sign and send the letters.

SIGNATURES:
Bolton Woods Way Septic Pumping Letter – The chairman of the Board signed the letter to be sent to all property owners on Bolton Woods Way.

OTHER BUSINESS:
Andrea Gilbert resigned from the Board of Health effective February 1, 2009.
